Filtering online pre-enrollment keeps us focused on what matters most to us… providing highly-attentive care to our patients.
After completing an online pre-enrollment form, you will receive direct communication from us by phone or email within 3 business days to schedule your first appointment. NO CHARGES ARE APPLIED to your card until verbally authorized by you after direct communication with the doctor to complete your enrollment.
Yes, it is also totally possible to get started the old-fashioned way, by picking up the phone and calling to schedule an appointment. If you’re less digitally inclined, we’ll gladly take care of the computer stuff for you. NO, we do not collect payment information when scheduling visits by phone, but we may ask for up-to-date contact and insurance information.
